I don't know where to start there are so many...

#1 has to be the floating/adjustable toolbars. I set them up when I first install Windows and NEVER have any need to ever change them again (mostly consists of turning off 99% of all the toolbars and/or buttons).

Of course, it is extremely easy (way too easy) with the tiniest flick of a mouse button (in error or without even realizing it) to totally wipe these things out. I then get to spend half an hour just trying to restore the fucking interface to the way it always was.

Just plain fucking annoying having to do this about once a week.

Hands down, "Microsoft Genuine Advantage". It's an invasive piece of DRM bloatware masquerading as something that's helpful. If you parse through their bullshit, the message is:

Great news! We've forced an update on you whose sole purpose is to hamstring your OS if you're not registered with us! If you have a pirated copy, this applies to you, but it can also ruin your day with a perfectly legitimate copy under the right circumstances. We consider this added hassle a good way to screw you out of the OEM installed OS that you paid for and force you to buy Vista. Click below to see more bullshit about how awesome we are for doing this!

User Account Control in Vista (asking for permission for every thing, had to switch it off).
